To File a Claim for Your Commercial Equipment
Please call your lender or financial institution directly to begin the claims process. One of their associates will discuss the loss information with you and assist you in completing any necessary claims forms. If the loss is due to a fire or theft, you will need to submit a copy of the report from the fire or police department, in addition to completing an Affidavit of Claim.
